Thank you for making your appointment at Allied Health Rehab Centers. We appreciate the opportunity to serve you.

The following information will be helpful in preparation for your visit.

• Be sure to bring all insurance cards.

• If your carrier requires a referral, please notify your doctor.

• Have prescription from referring physician.

• Plan on at least 1 hour for initial visit.

• You may want to bring a pair of shorts and comfortable fitting shirt or top depending on your diagnosis.

    Our goal is to provide the very best service and care to our patients "beginning at our front door and continuing throughout your rehabilitation."

You need to know:
   We have set aside one hour for this appointment. It is very important that you attend and be on time. A 24 hour notice is required for cancellation or you may be charged a cancellation fee.
